For insurance questions: If trying to use your health insurance plan during your appointment, please check our list of in-network insurance companies that we can accept before making your appointment. After checking the list, if you still have questions you can contact your insurance provider directly to ask for any additional questions you may have.

 

To make your first appointment or for our established patients who we have not seen for the past 6 months, please complete our virtual patient form and send it back to us. We will contact you within 24-48 hours to schedule your appointment.

If you prefer to contact us by phone, please call 971-413-3207 and leave a detailed message including your full name, insurance name and member ID number, medical reason for the call, and preferred days and times for your appointment. However, please keep in mind that completing the online intake form will allow us to provide you with expedited scheduling services. We are constantly checking our emails, and phone messages to provide fast services to all our patients.
